Studies of carbohydrate and lipid metabolism in women using the progesterone-T intrauterine device for six months: blood glucose, insulin, cholesterol, and triglyceride levels.

Abstract：

:A prospective, detailed study of both lipid and carbohydrate metabolism was carried out in 35 women who used a progesterone-T type intrauterine device (IUD). A 3-hour oral glucose tolerance test was performed in each woman before the IUD was inserted and again after 6 months of use. There was no change in the patients' weight or in the fasting glucose, cholesterol, or triglyceride levels. There were significant increases in the 3-hour glucose and the fasting, 0.5-, and 3-hour plasma insulin values in the 6-month tests. These values were less than those found after 6 months of systemic progestogen use, but do suggest some systemic effect of the released progesterone. :The carbohydrate and lipid metabolism was examined in 35 women using an IUD which releases small amounts of progesterone. Prior to insertion of the IUD, venous blood samples were obtained and a 3-hour oral glucose tolerance test was performed; 1 sample was taken before, the rest at 30, 60, 120, and 180 minutes after the ingestion of a solution containing 100 gm glucose. This metabolic test was repeated 6 months later. Each woman thus served as her own control. Weights remained unchanged. Fasting plasma cholesterol and triglyceride values showed insignificant changes. The 3-hour blood glucose value was slightly elevated. The fasting 0.5 and 3-hour plasma insulin values were significantly elevated, less than was the case in tests involving systemic synthetic progestogen treatment, but more than was the case with the use of inert IUDs. Some of the systemically absorbed progesterone thus seems to have a slight effect on the biologic activity of insulin.
