Overview Tumour Disease and Oncologic Therapy During Pregnancy.

Abstract:

:Newly diagnosed cancer in pregnancy is an issue with increasing importance, because fulfilling the wish to have a child is shifted later in life. Tumour disease is diagnosed in around 0.1% of all pregnancies in Germany. Most common tumour entities involve the breast, the cervix and the hematopoietic system. As tumour prognosis is not negatively affected by pregnancy and gestation does not disclude oncologic therapy it is important to plan treatment immediately. Oncologic surgery preserving the uterus is not at all limited. Chemotherapy with anthracyclines, taxanes or platinum derivates is possible from the second trimenon on. Only radiotherapy should be avoided during pregnancy. Before initiating therapy, fertility preserving methods like cryopreservation of oocytes and/or ovarian tissue should be offered to the patient. Preterm birth is more common in women with a first diagnosis of cancer during pregnancy. Other negative effects on the child's future are not yet known. Treatment of pregnant cancer patients should be handled by experienced centres with a multi-professional team. :Eine neu diagnostizierte Krebserkrankung stellt ein immer häufigeres Problem in der klinischen Praxis dar, da sich die Erfüllung des Kinderwunschs in den letzten Jahrzehnten in eine spätere Lebensphase verschoben hat. In Deutschland wird bei rund 0,1% aller Schwangerschaften eine Tumorerkrankung festgestellt. Die häufigsten Tumorentitäten sind Neoplasien der Brust, der Zervix und des hämatopoetischen Systems. Da die Schwangerschaft an sich die Prognose einer Tumorerkrankung nicht verschlechtert und keine Kontraindikation für eine onkologische Therapie darstellt, sollte unverzüglich nach der Diagnose die entsprechende Therapie eingeleitet werden. Onkologische Operationen, die den Uterus erhalten, sind während der ganzen Schwangerschaft möglich. Eine Chemotherapie mit Anthrazyklinen, Taxanen oder Platinderivaten ist ab dem 2. Trimenon möglich. Nur eine Bestrahlung sollte während der Schwangerschaft vermieden werden. Vor dem Therapiebeginn sollten der Patientin fertilitätsprotektive Maßnahmen wie eine Kryokonservierung von Eizellen und/oder Ovargewebe angeboten werden. Frühgeburten treten häufiger bei Frauen mit der Erstdiagnose Krebs während der Schwangerschaft auf. Andere nachteilige Effekte von Erkrankung und Therapie auf das spätere Leben der Kinder sind nicht bekannt. Die Behandlung von an Krebs erkrankten Schwangeren sollte an spezialisierten Zentren durch ein multi-professionelles Team erfolgen.
